Synergen Behavioral Health, PA is required by law to maintain the privacy of clients Protected Health Information (PHI) and to provide clients, with this NOTICE OF PRIVACY PRACTICES that describes how we may use and disclose PHI and your rights to access and control your PHI.
PHI includes all individually identifiable health information transmitted or maintained by Synergen Behavioral Health, PA, regardless of form (oral, written, or electronic). Your protected health information is covered by the HIPAA Privacy Rule and applies to a health care provider, health plan or health care clearing house that relates to physical or mental health conditions, provision of health care, or payment for health care, whether past, present, or future.
